While I was reasoning through all the options for underwear, I found these and thought that they would be perfect for post-op stage 1 and possibly after glansplasty. I say this based on surgeon preference, of course; my surgeons told me I could wear regular underwear as soon as I came home as long as I still propped and used my little cloud of gauze. I am putting two different options: one with the option to tuck your dick back into the pouch and one where it would just hang freely, because I am not sure which one I am going to get, but I am going to try one of them just to see and I will report back after I get them. Also here is the link if anyone is interested

I had RFF phallo 3 wks ago yesterday. Already had meta with UL, V-ectomy and scrotoplasty so it was just finishing UL, phallus creation and a temp urethra between dick and balls. Ill get that closed and stp at stage 2. Had some superficial separation at the base of my shaft and some minor tip necrosis after some blisters while in the hospital. All is healing very slowly but still healing good. And up until today the smell of sloughing skin, blood and leaking urine was horrendous and very bad on my self esteem but it finally stopped! Next post op and possibly last or 2nd to last is Monday! Only up from here boys!
Edit: I forgot to add that i also had burial done this stage. Thats why I had to get the temp urethra.

While supply shopping for surgery I found these and had a little chuckle, but I’m curious if anyone used these while healing and if they’d recommend ‘em.
I thought these would be great especially for the initial healing when needing a pillow.
Open to other recommendations as well!

I’ve done all steps of alt phalloplasty and I’m a bit over 1 year post op. I’m super happy and even though it was a rough couple of years I don’t regret going through with it.
The only thing that gives me massive dysphoria still is sexual encounters or discovering my own body. I have a girlfriend and she’s been amazing throughout but I feel like our sexual life has really taken a toll. I have some feelings in my dick but I don’t feel much during oral sex or penetration which is obviously a little disappointing (even though expected) and it’s very difficult for me to orgasm without touching my buried t-dick.
I’m trying to go through the steps of rediscovering my body and more importantly accepting the changes. I was thinking of maybe using toys to facilitate pleasure when I masturbate and/or have sex without actively touching my t-dick.
I was wondering if anyone had any recommendations for toys safe to use after phalloplasty. Also I would love to discuss about your general experience regarding sex post surgery and how you acclimated to your new body.

I'm planning on surgery with Dr. Chen, currently in the process of trying to get a consult. I am a self-employed independent contractor currently on Medicaid in a different state. I make around $24k a year, sometimes less, but have a lot saved up. Before my surgery, I want to buy marketplace insurance that Chen accepts, like BCBS, but I'm worried about being out-of-state and whether or not I'll qualify based on my income. I really don't know how any of it works. Has anyone else attempted this? Any success stories/warnings I should know?
In terms of alternative methods like job-based coverage or Medi-Cal, there's no guarantee that I'll be able to find a job in my state that Chen will accept, so I'm not counting on it. I also don't think I'm willing/able move my whole life to California, where I have zero network, family, or community. But nothing is completely off the table at this point.
Would love some guidance or other peoples experiences with insurance. This stuff is really tricky!

I am many years post full meta with UL, vnectomy, vy scrotoplasty, implants. I'm seeking out phallo now.
My sack currently sits between my legs. This is a huge concern of mine in terms of aesthetics and comfort. It is both physically uncomfortable, and dysphoria inducing. I am interested in getting some sort of significant scrotal lift, or just having the scrotum redone entirely when I get phallo.
I did have some upper scrotal tissue removed in my stage two, so I understand that my sack will likely be smaller than had I gone straight for phallo. Yes, this upsets me. But it is what it is. I just really want my sack to be positioned in front of my legs and I want "good" positioning of the phallus.
I am working on stretching my scrotal skin to hopefully accommodate larger implants and some repositioning. However, I am worried about making any skin too thin to be able to safely operate on. Does anybody have experience with scrotal stretching and/or repositioning a meta scrotum when getting phallo? Can the skin be made so thin that it cannot be operated on?

I'm a few months post op RFF, and I haven't been doing my hand PT, lymphatic drainage massage, or scar massage as often as I should. I'll do it for a handful of days, and then will stop for a week due to mental blocks, distractedness, laziness, etc. Each day I forget/fail to do it, I catastrophize a little.
I think I'm trying to put my mind at ease by asking: Has anyone NOT done much PT and scar massage and ended up pretty happy with their scar appearance, hand swelling reduction, and wrist mobility years later?

I am sure this has happened to other folks before and I am sure this is common but when I searched it in the group (thanks Reddit for your horrendous search features), I didn’t see anything similar. I have RFF scheduled for December. Pre authorization was submitted with 33 codes. I received an email from my case manager “The authorization has been determined as partially approved, there are 11 codes denied and 22 approved.” So I guess 66% of my surgery is approved and now I just gotta hope they can duke out the last 11 codes??? Has this happened to people before and if so, did it resolve itself? I have been hesitant to book accommodations as insurance has been horrific for me and I’m worried these denials will get my surgery cancelled.

I am 6wpo rod(and other stuff) and have encountered a problem. I am going back to work in a couple weeks, and I wear scrubs so not super forgiving in terms of bulge prominence. I had ALT so already pretty girthy, used to wearing snug underwear and that was always fine before, but now i am concerned about my underwear putting pressure on tip and risk of erosion.
Does anyone know of any underwear that will flatten without lifting too much? I am about 5” long and 6-6.5” circumference

Currently scheduled with RBL for no UL but leaning towards wanting UL and RBL doesn’t do this w/o hysto + vnectomy. So far this is who I’ve found that does it- any one to add?

Hey yall. I had stage 1 RFF (phallus creation, UL, vaginectomy, & scrotoplasty) at the end of July. But ya boy has been dealing with a fistula at the base of my phallus. It’s been closing and reopening since about week 7ish.
My question is, have any of yall dealt with this? If so, do you have any tips to get it closed or is it more so gonna have to be fixed in stage 2?

Getting tattooing with Evolve Studio in January :) took these photos for the artist. I'm about 5 inches long stretched, not sure how big around currently but probably around 4-5 inches. At least I was when I was still a little swollen lol. Next stage is testicular implants, then pump later after tattooing. My surgeon was Dr Ali Salim with Kaiser SF

Here’s my update with the pump, it’s been 6 weeks since I got it. The recovery was pretty easy in comparison to the rod, but maybe that because there was already the space to put in the device. I haven’t been cleared for sex yet so can’t comment much on that, however I do feel that when fully pumped the pump is firmer than the rod was. I was worried at first using the pump, especially deflating, would be hard but it’s actually pretty quick and easy once you learn where the button is lol. Overall I’m loving the pump so much I don’t see a reason I’d ever go back to the rod. I’m very active so being able to be flaccid when I’m at the gym has been so great. I still have a bulge but it doesn’t feel as obvious which I like a lot. I feel like it’s easier to wear tighter clothes and there’s not constant pressure on the device like it was with the rod.

When did the discomfort and swelling easy up for yall after surgery for the pump? I am 12 days post op and shit I thought by now I would atleast be able to move around a little better but that is not the case. My discomfort and issue is with the bulb and the whole left side.
I did reach out to my clinic and they said it’s probably just due to my significant swelling and bruising. To continue to ice and take my anti inflammatories.
Any advice or making sure I’m not crazy and this is normal would be great lol.

Hey all. It’s been awhile! I had my final stage (ED placement and left ball implant? I think lol) in April of 2025. I had medical tattooing at Evolve on August 8th.
Medical tattooing helped me so much in feeling closure around dick journey. I feel a lot more connected to my dick and am hopeful that’ll continue.
I went through a breakup about a month ago and was in a relationship where we didn’t really have sex. I haven’t tried too much yet in terms of sex, but I’m feeling hopeful I can branch out and try more as I feel up to it.
Sometimes I still wish I was bigger, but I have learned to feel a lot more happy about where I am at and I am so grateful to have made it through to the other side. I am finally at a point where my dick is just my dick, and that’s all I ever wanted.
I measure about 4.5 x 4.5 inches for length and girth. Here are some pics of where I’m at flaccid and hard :) to anyone who is just starting this journey, it is truly a tough one. Be graceful with yourself and remember to reach out to community ❤️

Hi everyone,
I'm currently in the process of scheduling and initiating my ALT phallo journey and I've been thinking more about my options. I'm currently scheduled for stage 0 (vaginectomy & donor site flap lift) delayed ALT with the Crane Center in Boulder in January 2027, followed by stage 1 (phallus creation, UL, scrotoplasty) in July 2027.
Like many others, I have always been particularly interested in receiving care from the Buncke clinic/Chen/Watt/Safa. If I understand correctly, they can sometimes expedite their services if the first stage of phallo is completed with a different surgeon. I would be particularly interested in receiving my glansplasty and implants with their team.
Has anyone else received the first stage from one surgeon and then went with another for the secondary/aesthetically-aligned surgeries? Did you run into any issues with your insurance? Are there drawbacks to doing this that I may not be considering fully?
Would love to hear from anyone with experience doing this.

Link to my 2 week update here
TLDR: recovery has continued to go smoothly 🎉 I have some wound separation in my perineum, but it’s slowly getting better each day. I’ve been peeing from my penis for the last 5 days with no leaking from the wound separation and no fistulas. The SP catheter was removed 2 days ago at my last post op appointment.
I had my second post op appointment about a week and a half ago (19 days post op). During this appointment, they took some photos, changed my arm bandaging, removed the remaining staples from my arm, and gave me instructions for my voiding trials.
At 24 days post op, I started voiding trials. The first couple tries were difficult - had a hard time getting started and less than half of my bladder was able to be emptied through my penis. But by the of the day, I was able to empty 100% through my penis. It just took some time to remember how to pee and redevelop the mind-muscle connection. After just a few days, it’s already feeling so natural.
I had my third (and last) post op appointment about earlier this week (at 26 days post op). During this appointment, they took some photos, changed my arm bandaging (and switched me to just a dry gauze dressing), and removed the SP catheter. I was worried getting it removed was going to be painful but it wasn’t bad at all, just felt like some pressure. The SP insertion site was really sore that night and the next day, but is hardly noticeable today.
I’ve been in PT for my hand/arm weekly. I’m feeling really good about the strength (probably 80% normal). I’ve made progress with range of motion, but still have a long way to go to get back to normal (probably at 50% right now). That being said, I’m able to cook, clean, type on the computer, text normally, etc now. My hand is still very swollen. I’ve been trying to keep it elevated as much as possible and do lymphatic drainage massaging which seems to be helping some.
The xeroform on my thigh finished peeling off on my 16th day post op. Since then, I’ve been applying aquafor once a day. The skin itches some, but it’s not constant. There’s a couple areas that are blistering now, but nothing too crazy.
Pain level is basically zero now. I’m only taking ibuprofen occasionally. I do have some discomfort and itching sensations around the sutures at the base of my penis, usually when I’ve been walking around and dangling for an extended period of time.
Finally after 2 cancelled surgeries July 24th 2026 and August 27th 2026. Time to be BRICKED UP lol😂 finally after having phalloplasty since October 2023 having 4 complications 3 strictures and 1 fistula I can finally say tomorrow morning at 7:30am I will be getting the Titan Coloplast Pump! Wish me luck brothers and pray for ya boy!!!! We finally made it to the end of the rocky road. Always remember there is light at the end of that tunnel these complications are fixable and patience is KEY!

Hello lads,
Anyone else just got the email saying their referral is being transferred to C&W? I sort of assumed this would be the case when they told us that NVH would no longer offer it. That in itself has thrown my head into chaos for the past however many months since finding out. I needed a revision and had a surgery date, but had another medical problem that required investigation. Now the investigations are done, but NVH won’t be there anymore. I cry every day.
Anyway onto my questions. Anyone have any information about if the surgeons at NVH will be going to C&W or are we straight up losing the main phallo surgeons? I heard C&W have only done about handful of surgeries, I’ve not seen results, I just hear about inexperience. this whole thing has been so hard already, just to be honest, if they fucked up my body I would probably just end it, ya know? Not strong enough for this shit.

I'm getting electrolysis only on my UL area but I've seen contradictory information about needing to remove the light/blonde hairs. As you can hopefulyl see in the pictures, some of them seem longer than what I would consider peach fuzz. Should I be asking my electrologist to remove those ones as well? Currently he's only removing the darker, longer hairs.
Had a 45min treatment session earlier today, haven't been shaving.
