I'm a 33-year-old male in the UK, 6 ft, 83 kg, not overweight and I go to the gym. I've had persistent low libido and virtually no morning erections, alongside low mood, brain fog, low motivation and some anxiety/overthinking.
I'm waiting for an NHS endocrinology appointment, but it's still around 6 months away, and I've already been waiting a while. I'm looking for possible causes to investigate or sensible things to try in the meantime, rather than just sitting around waiting. Appreciate any help
My blood results so far:
Total testosterone: 12.90 nmol/L (8 am, March 2026; range 8.6–29). Previous result was 11.80 in December 2023.
Prolactin: 178 mIU/L (range 61–257).
TSH: 1.07 mIU/L; TPO antibodies normal.
Folate: Previously low at 3.0, increasing to 7.5 after around three months of folic acid. This is the only documented deficiency on my blood-test paperwork.
B12: 559 previously while supplementing.
Ferritin: 148 µg/L.
HbA1c: 36 mmol/mol.
ALT: Improved from 51 to 25 after reducing alcohol.
Triglycerides: Previously 2.18 mmol/L, now 0.92 after dietary changes and reducing alcohol. Latest total cholesterol 5.1 and HDL 1.28.
I haven't had SHBG, calculated free testosterone, LH or FSH tested, and I have no sleep-tracking data to assess my sleep quality.
I'm keen to find potential underlying causes and make progress while waiting, rather than jumping straight into testosterone treatment.
Any advice or personal experiences would be appreciated.
