Hi,
Since migrating to a new iPhone today, the 1Password AutoFill sheet is blank every time. Tapping "Passwords" above the keyboard, or choosing a passkey, instantly slides in an empty black or white sheet. It happens in every app and browser, for both passwords and passkeys. The main 1Password app works normally, and Apple Passwords and other password managers fill fine on the same fields.
All my passkeys and OTP codes are in 1Password, so I currently can't sign in to anything that relies on a passkey on this phone.
Device and versions
- iPhone 18 Pro Max (iPhone19,7)
- iOS 27.0.1 (24A446)
- 1Password for iOS, latest App Store version
- Set up by restoring from an iCloud backup ("Download from iCloud")
Examples
- A game's Google sign-in offers my passkey as the only option. Tapping Next brings up the blank sheet.
- Reverse 1999, "Login with password": tapping "Passwords" above the keyboard to generate a password brings up the blank sheet.
Already tried
- Reinstalled 1Password, Chrome and the Apple Passwords app
- Changed the default browser between Safari and Chrome
- Removed and re-added the Safari extension
- Turned each 1Password security setting off and on, including Face ID
- Force quit all apps involved, restarted the phone several times, cleared caches
- Turned AutoFill Passwords and Passkeys off and on, and tried every combination of providers
- Everything is up to date. No Screen Time restrictions or Lockdown Mode.
What the device logs show:
No crash report exists for 1Password or its extension. A JetsamEvent captured while reproducing the issue in Reverse 1999 (2026-10-11 22:06:39, incident 941B65F3-6702-43EC-959F-7ACD2ACAFCCB) shows:
- Two instances of "Password AutoFill Extension", both active at priority 100, neither terminated:
- pid 614: about 33 MB resident (peak about 41 MB), 3.29 s CPU, long-lived from an earlier invocation and still active
- pid 1332: about 22 MB resident (peak about 38 MB), 0.40 s CPU, the current invocation, in the same coalition as CredentialProviderExtensionHelper pid 1331
- The main 1Password app (pid 1344) suspended in the background
- The only processes killed were Apple daemons (pfd: vm-compressor-thrashing; appstorecomponentsd: highwater)
So the extension launches and stays resident but renders nothing, and an earlier instance never exits. My guess is that it can't reach vault or keychain data after the iCloud restore.

I can send the full log or a diagnostics report if that helps.
