Because he's extremely mentally ill, and probably whatever stimulants they have him on to stave off slowly advancing dementia and keep him awake throughout the day just exacerbates his narcissistic personality disorder and grandiosity.
He flat out said the only mistake he made during his first presidency was not having loyal people around him. He doesn't want to be challenged, he wants zealous sycophants.
