Title: How can I pursue BSSO after correcting my deep bite with Invisalign? (LA area)
I’m looking for advice on how to pursue BSSO (bilateral sagittal split osteotomy) after correcting my deep bite with Invisalign. I’m concerned that my lower jaw and chin are somewhat recessed, and I want to find out whether mandibular advancement could be an option for both functional and aesthetic reasons. I also have UARS (upper airway resistance syndrome), and everyone in my family has sleep apnea, so I’m worried that my jaw position could put me at risk of developing sleep apnea in the future. I live in the Los Angeles area and would appreciate recommendations for experienced oral and maxillofacial surgeons who evaluate patients with possible mandibular retrusion and sleep-disordered breathing, including borderline cases where the bite has already been corrected orthodontically. Should I consult an OMFS directly or start with an orthodontist who specializes in surgical cases? How can I get an objective evaluation of my jaw position and airway, and what imaging or sleep-study results should I bring? I’d really appreciate advice from anyone who has been through a similar situation.

Hi, I’m wondering if it’s possible to have asymmetrical jaw without misalignment in teeth. It’s weird because my lips don’t align naturally even though my teeth do. I think that’s what I might have but have not yet gone to get x ray. So was wondering if anyone could answer before I go

Advice for Me How do I bring up possible jaw hypermobility to my surgeon?

I qualify for surgery and I’m currently in the process of beginning decompensation. Reading posts and the experiences of people on here has ‘clicked’ in my brain that I may have jaw hyper mobility.

I experience severe muscle fatigue and never pain in my jaw to the point that I’ve almost pulled out teeth on accident by pushing onto them too hard during flare-ups. The main reason it clicked though is that J am able to move my jaw in ways other people are unable to and can open my mouth by (measured by an orthodontist) ~82 mm.

I’m only 18 (I have a severe facial deformity and even had distraction osteogenesis done as a child along with other measures such as palette expanders. Please do not come at me with “Just try X!”) so issues like joint degradation, etc. wouldn’t be around right now, but I am concerned about future problems.

Does anyone have tips on how I could discuss this with my surgeon?

36M – Severe sleep apnea has destroyed my life. How much advancement can my face realistically handle without looking unnatural?

I'm a 36-year-old male, and severe obstructive sleep apnea (ahi 39)has completely destroyed my quality of life.

For years, I've struggled with terrible sleep, constant awakenings, choking sensations, and the feeling that my airway is collapsing every time I start falling asleep. It has reached a point where I can barely function normally anymore. My entire life revolves around trying to breathe and get some decent sleep.

I recently underwent a DISE (Drug-Induced Sleep Endoscopy), which confirmed multilevel airway collapse, including:

Complete (100%) tongue base collapse

Secondary epiglottic collapse, caused by my tongue base pushing the epiglottis backward

Additional obstruction at multiple levels of my upper airway

I've tried pretty much everything I can think of, including:

CPAP, including experimenting with different pressures and many masks

Mandibular advancement device (MAD) which really did a big improvement but many side effects/pain/headaches

Myofunctional therapy

Various breathing exercises and positional adjustments

Unfortunately, nothing has provided a sustainable solution.

I'm now seriously considering maxillomandibular advancement (MMA) surgery with counterclockwise (CCW) rotation, specifically to address my severe airway obstruction

At this point, my priority is being able to breathe and sleep normally again. However, I'm also concerned about the aesthetic consequences of large skeletal advancements.

My questions are:

Based on my facial structure, how much maxillary and mandibular advancement do you think I could aesthetically tolerate before looking unnatural?

Would CCW rotation potentially allow for greater airway improvement without excessively advancing my midface?

For those who had complete tongue base collapse and secondary epiglottic collapse, did MMA resolve these issues?

Would you prioritize maximum airway expansion even if it meant compromising facial aesthetics?

I'm not expecting anyone to replace a qualified maxillofacial surgeon. I'm simply interested in hearing opinions and experiences, especially from people who have undergone large advancements.

I genuinely feel like I've lost my life to this condition, and I'm desperate to get it back.

I don't care about achieving some perfect model-like jawline. I just want to breathe normally, sleep through the night, and still recognize myself in the mirror.

Any advice or personal experiences would mean a lot.

Advice for Me i hope that i need jaw surgery

I’m suffering immensely because of the way my jaw makes me look, and I’m sure there’s something wrong.

From the front, everything below my nose looks elongated, especially the area between my nose and mouth. My entire lower third completely throws off the harmony of my face.
I have a convex profile, and my nose looks long.
I also have dark circles, but it’s not just due to pigmentation. I have nasolabial folds as well, which I never used to have. I’m 18, by the way.
I’ve been wondering whether all of this could be due to the fact that I used to breathe through my mouth a lot. I don’t know exactly when it started, but I became aware of it when I was 14.
This year, I was diagnosed with a deviated nasal septum.

I’m planning to make an appointment with an oral and maxillofacial surgeon to find out whether I actually have a jaw problem. I don’t think I have any symptoms apart from the way my face looks.
As I said, I’m suffering so much because of this insecurity. It’s awful. My biggest nightmare is being told that surgery isn’t an option.

btw i had none of these problem when i was younger, i looked so much better

Finally

Hi y'all! ❤️

It's been a while since I last posted here. I honestly almost gave up on my jaw surgery journey, but today something happened that gave me hope again.

I went to the orthodontist and told her my story about how I didn't feel taken seriously during my consultation with the jaw surgeon. I felt so defeated afterward. To me, the problem was so obvious, yet I felt like they were so cold and dismissive about everything I was going through.

But today, I met the sweetest woman at the orthodontist's office. I told her everything, and she couldn't understand why I had been treated that way.

She told me, "If you want, we can make a treatment plan and work towards getting you the surgery you need."

Of course, I'll have to pay for the braces myself, around €3,000, but I already knew that might be part of the process.

Honestly, the money wasn't even what mattered most to me in that moment. It was the fact that someone finally listened, took my concerns seriously, and was willing to help me move forward.

After all the pain, insecurity, and bullying I've been through because of my jaw, I can't even describe how relieved I felt. 🥹

I know the surgery isn't officially confirmed yet, but for the first time in a long time, I feel hopeful again.

So I'm back, and I'll keep you guys updated on my journey!

And to anyone who's feeling hopeless or unheard: you are not alone. Sometimes, finding the right person to listen can make all the difference. Keep advocating for yourself, asking questions, and seeking the answers you deserve. ❤️

Today is exactly one month since my bimax. Today I saw my orthodontist for the first time and she also recommended for me to go for a second opinion to an oral surgeon for the asymmetry and whether my maxilla is centred. I’m still worried that I look worse now due to the look of my upper lip, nose and because I feel like my upper jaw was moved forward however lower stayed recessed and downward, but I also understand that there’s still swelling. Last 2 are before surgery. Does anyone have any good recommendations for surgeons or hospitals for revision surgery in the Netherlands, I would appreciate some advice.

Advice for Me Smile still shallow after MSE, Double jaw surgery and invisilign. Need advice + solutions please

Thumbnail

I've had MSE expansion which expanded my upper palate from 34-39mm, double jaw surgery which moved my upper jaw forward 10mm, and currently in the middle of invisilign where I have elastics to extrude my upper molars lower. I feel like my smile still feels extremely shallow and my back teeth and canines are too high.

My incisors are also still slightly flare as my upper jaw still feel recessed so they flare to compensate. I'm genuinely so frustrated having tried so many options to achieve a genuinely full smile as close as I can to the last few photos as reference. I unfortunately had premolars removed when I was a child, and this has been my long journey to try to not only reverse that, but gain a full smile.

Please are there any orthodontic or other options that could project my upper teeth, completely upright them, lower my molars and canines significantly. I've looked into SFOT but unsure how that would work with my short roots and screws in my upper jaw from DJS. I'm looking into veneers as a last resort but after all of this effort I want to really pursue any intervention to achieve as much fullness as I can with my natural teeth. Any and every advice is appreciated.

Advice for Me Will slanted jaw heal with time?

Thumbnail

Hi all, I had emergency jaw surgery back in July after falling and breaking my left mandibular condyle. My surgeon installed hardware and banded my mouth shut to help realign my left side while it heals. I’m 3 months post op and 90% normal with little to no pain when eating normal foods (though still difficult to eat super hard and chewy foods), and currently working on being able to open my mouth all the way again as my healthy non broken side has gotten tight after zero to little use.

My jaw is still a bit crooked when I open my mouth, and when talking I can definitely feel the back of my left (broken side) teeth hitting my top in a way my right side doesn’t. My teeth align fine when closed but the opening is crooked, with the left side (broken) shorter than my non broken side.

Has anyone experienced something similar and had their jaw heal fine? My surgeon said the jaw joint takes about 1 year to fully remodel, but I’m not feeling super hopeful that my left side will go back to normal.

Thanks for any advice!

Advice for Me How to overcome the fear of jaw surgery

Hello !

I am a determined yet completely afraid girl, and I need feedbacks !

A few years ago, I was told I needed jaw surgery (my lower jaw is recessed). I was so afraid that I refused it completely and just carried on with my life.

But the ortho who recommended it warned me that my teeth were more likely to wear down and my gums to recede, so I always kept it in the back of my mind, thinking I'd eventually need it, just later.

Now that I'm almost 30, I'm sick of not doing something good for myself out of fear ! I want to stop feeling insecure about my profile, and my gums are starting to feel very thin around my front teeth.

I think it's time, but I'm freaking out. I have a surgeon's appointment in november and I can't stop thinking about it. I already had my ortho appointement with x-rays and impressions.

My ortho said I'd need four premolar extractions to decompensate, because my teeth are very flared out.

Having four teeth pulled and then undergoing surgery feels like such a life-changing experience, and I'm terrified. I'm scared of the anesthesia (the loss of control), of having my jaw wired shut, and of the pain. I am starting to "accept" Lower jaw surgery but if the surgeon tells me I need DJS I might freak out even more.

Every time I see someone who's had JS, I think they're a superhero and that I'm absolutely not capable of doing the same !!

I feel i'm going crazy because here in France dentists and ortho keep telling me it's nothing, it's a very basic and well-done surgery. While i don't doubt that surgeons do it very well multiple Times a day, it's not my understanding that on the patient POV it is something to take lightly... Some people even say it is the most challenging thing they have done. I feel like doctors, even very sweet ones, dont understand the fear... And that makes me feel bad and not very brave.

Has anyone been through this surgery after being very scared at first ? Did you have a trigger/wake-up call to finaly start the process or did you start by ignoring the fear and jump ?

DJS for underbite: I’m terrified

Thumbnail

I’ve been going through the process of braces with the eventual outcome to have double jaw surgery to fix and underbite (on the NHS in the UK). I had a routine braces check up appointment today and was advised that I could be getting my jaw surgery march 2027 which after a 4 and half year journey was abit of a shock.

I’m not massively insecure about my underbite but it does cause some functional issues. I’m terrified of the getting the surgery. I’m scared of how it’s going to change my face, I know the change to my jaw will be positive but the changes it can cause to the rest of my face (nose and symmetry) scares me. I know it sounds daft but I’m scared that I won’t like my new face and that my boyfriend won’t like it either (daft I know). With me not being massively insecure the thought of change is scary because I don’t necessarily want to look too different.

My question to you is would it be worth it?

The photos I have attached are photos I e taken of my self and photos other people have taken of me. I feel like in selfies you can barely notice it. But in photos other people take you can? It’s so distracting not knowing how you look!

Advice for Me Do you think i got enough advancement?

Thumbnail

I am around 6mo post-op bimax where I got the following advancements:

Upper jaw: 2mm forward, 3mm posterior downward
Lower jaw: estimated 9/10mm movement (my underbite was 10mm)

Keep in mind that the x-rays are taken from different machines and head angles.

I'm planned to get a genio in about 6mo but I'm not sure whether it is the appropriate answer.

Advice for Me Insurance wants to deny surgery

I have been referred to see an Orthodontic surgeon but was told that my insurance most likely would not cover a DJS.

How can I communicate properly to my doctor in order to be approved for DJS?

My phone consult informed me that they want to adminster Botox Injections, which I am not a huge fan of, but am willing to try if it relieves the pain.

My symptoms:

-Teeth grinding

-Grind holes through my retainers

-Jaw Clenching

-Jaw pain

-Can't touch teeth on left side of face

-Can be hard to eat & breathe, especially when running or sleeping

-Jaw sometimes makes popping sound on right side of jaw, clicks when it opens

-Lopsided smile , suspect a cant due to crossbite

Advice for Me Currently undergoing orthodontics before DJS surgery next year & feeling ugly :(

32F I feel like I’m delaying or can’t have things in life until I get my jaw fixed. For example, I’ve been wanting to post creatively online, but am very afraid of internet trolls commenting on my jaw. If anything, I also know I will feel more confident once it is fixed, and put myself out there when I’m happier with my appearance.

I have a severe underbite, and going through decompensation is making it worse. I already got SARPE this year and had to live with being insecure about a giant 4mm gap. Thankfully that has since closed. My orthodontist said I can expect to get my surgery for late Spring / Summer of 2027. I know time will past anyhow, but feeling especially insecure lately and want to get it done sooner.

Yesterday, a stranger on a dating app commented negatively on my jaw, and how he wasn’t sure if I was a man. 😭

Question: How did you psychologically stay in a healthy mindset before your surgery?

Underbite and crossbite, being told to get double jaw surgery

Thumbnail

I have an underbite and crossbite. Two different orthodontists and OMFS have said I should get double jaw surgery to fix my breathing (I can’t really breathe through my nose right now), prevent future tooth wear, fix bite, aesthetics/facial symmetry and speech

They want to do a 7mm upper jaw movement forward (3 piece Le fort to widen) and a 2mm lower jaw backward (and to fix the crossbite)

But honestly I think I could live the rest of my life like this (this isn’t life threatening) so I’m struggling with this decision. Has anyone else been in this situation, based on my teeth images what do yall think?

I got jaw surgery from an injury and I’m about 4 weeks post op. I had two plates where the swelling still is and I can feel the hardware. Will this go down or would I need to remove hardware for it to be unnoticeable and back to normal? The swelling was intensely bad pre op and post op but most of it went down. Now just left with this bulge.
