Insurance/PA Zepbound for OSA Medicare part D Massachusetts

• Upvotes

Silverscript is leaving Massachusetts so I have to find a new Medicare Part D plan that covers it for obstructive sleep apnea. I’m NOT looking for the bridge program. I’m not seeing Zepbound in ANY of the 2027 formularies.

First Timer Wegovy to zep

• Upvotes

I was on wegovy from June 2025 up until 2 weeks ago. Once I saw zep was back on my formulary I asked to switch as I do have obstructive sleep apnea. On wegovy I didn’t lose weight until I made larger changes to my diet and exercise in January and since then I had lost about 45 lbs.

I went from 2.4 wegovy to 5 zep. I didn’t have many side effects on wegovy, and so far after my shot yesterday evening I’m not having any side effects.

I’m so excited for this journey! I feel like wegovy was very hit or miss on whether I got full fast, which always really confused me. I also still had massive sugar cravings, so hoping the switch could help me there too.

Vent/Rant Puzzled

• Upvotes

I'm in my second month (6th week), still on the 2.5mg dose because I feel very strong hunger/cravings suppression. I have been losing ~3.3 lbs per week but this week the scale is going up and down instead of down like before and I don't think I will lose any weight this week (or maybe the weight will go up a bit compared to last week!). I'm eating the same way as the weeks before.

What is going on? I was so excited about the journey and I'm now confused and worried that it won't work :(

Dosing Should I keep increasing doses?

• Upvotes

I’m in the military so we don’t really have doctors at my base just RNs and they treat you more like a military than a patient. I’m curious to hear from those who have had no side effects, what’s you and your doctor’s approach to increasing?

I’ve been on Zep since Aug 4th and I’ve lost 27lbs so far from 218lbs to 191lbs. I am currently on 7.5mg and I’ve had zero negative side effects since starting.

I have a phone call with my healthcare provider every 4 weeks and she usually ask me if I’ve had any side effects and when I say no, she asks if I want to increase. I once asked her if I should increase and she said she didn’t know and that it was up to me. Our calls usually last less than 2 mins.

So far, I’ve been saying yes because we are only allowed to take it for six months so my logic was to get as much out of it as possible. However, I’ve been reading lately that higher dosages doesn’t necessarily mean better. I have my call with her next week so I don’t know if I should keep increasing.

Dosing Shorter time between shots or skip

I am on shot 7. Still at 2.5 mg and losing weight. I have been traveling and missed the due date for shot 7. It was due on Wednesday. It’s now Friday. I can take it tonight and still take it next Wednesday. Better just to skip? Shot 6 had a bunch of GI side effects and wiped me out. No prior side effects.

Dr. said I could do either.

Thanks.

Personal Insights 46F, on Zepbound 15 mg since January, only lost 15 lbs. Anyone else struggling, especially after gastric sleeve surgery?

Hi everyone!
I’m hoping to hear from people who may have experienced something similar because, quite frankly, I’m frustrated and feeling defeated.
I’m a 46-year-old woman with PCOS, currently going through perimenopause. I had gastric sleeve surgery in 2014 when I was 34 and lost approximately 115 pounds within the first year. I maintained my weight loss for about 10 years before gradually gaining back around 40 pounds.
I started Zepbound on January 8, 2026, weighing 217.3 pounds. I’m currently on the highest dose, 15 mg, and as of today, I weigh 201.8 pounds.
That’s only 15.5 pounds lost in a little over nine months.
At one point, I got down to around 196–197 pounds, but I’ve been bouncing around the same few pounds for months. It’s like my body refuses to stay below 200!
I don’t eat large meals. In fact, I mostly nibble throughout the day and often can’t finish what I prepare. I also want to be transparent that I’m not currently following a regular exercise routine.
I know everyone responds differently to medication, but it’s discouraging seeing people lose 40, 50, or even 75 pounds while I’m struggling to lose 20.
I’m paying approximately $450 for this medication, and I’m starting to question whether it’s worth continuing.
My doctor previously suggested Wegovy, but I’ve been hesitant to switch because I’ve read that Zepbound generally produces greater weight loss. I’m also worried about regaining weight if I change medications.
I realize I’m not 34 anymore, and my body has changed. But I never imagined weight loss would be this difficult after having such success with my gastric sleeve.
I’m genuinely feeling like a failure, even though I know I shouldn’t compare my journey to anyone else’s.
My questions are:
Has anyone had gastric sleeve surgery and later struggled to lose weight on Zepbound?
Has anyone experienced a prolonged plateau on 15 mg and eventually started losing again?
For women in their 40s dealing with PCOS or perimenopause, did anything help?
Has anyone switched from Zepbound to Wegovy and actually had better results?
If you were in my position, what would you discuss with your doctor before spending another $450?
I’m not looking for a miracle or a quick fix. I just want to understand why my progress has been so slow and whether there’s something I should be doing differently.
Please be kind. I’m already pretty hard on myself about this.
Thanks in advance for sharing your experiences!

Insurance/PA Don’t give up the good fight

I finally got my Zepbound sorted out with insurance. I have been on self-pay since April 8th when I started. First month 2.5mg vials, then three months of 5mg kwikpens, two months of 7.5mg kwikpens, and now two months of 10mg kwikpens(including one I just picked up a couple days ago that I havent used yet).

My doctor has probably put in Prior Authorizations over the last year (was trying to start last October when my first sleep study results cane in and finally gave up and did self-pay). I have severe OSA and insurance denied me every time. A few months ago I had the in lab sleep study which also shows OSA. Dr tried again with a PA. Still denied. When I went in to plead for a dose increase lazt month (to 10mg) she said she was gonna do another one. I laughed and said don’t waste your time. They won’t approve it. She said we have to try. It could slip through the cracks.

Well, two days ago I got a letter saying it was approved. Weirdly, the letter was dated 9/22 but just showed up 10/13. Anyways, it was a cluster$@&@ trying to get everything sorted still. Spent an hour and a half over yesterday and today with insurance, doctor, pharmacy, and the company the insurance outsources the pharmacy stuff to. But ultimately, i ended up at $25 dollars per month (at least until the PA expires in March) i was adamant it wouldn’t be covered so dont give up hope ladies and gentlemen. I love the kwikpen but not at that price difference!

Dosing Suddenly craving sweets?

I’ve been on Zepbound since March and I’ve lost about 30 to 35 pounds. I’d like to lose another 10-15. It feels kind of slow but I don’t mind because I assume it’s healthier and might stay off. Since starting, I haven’t had too many side effects that I’ve also had almost no appetite and it had to work to get in enough protein, and calories in general. One nice thing was that my sweet tooth was really reduced. However, this week, my eighth on 7.5, I started craving sweets a little bit and then today I went absolutely bonkers and ate a whole packet of cookies. I’m postmenopausal so it’s not that it was so weird since it’s been so long since I’ve had any desire for cookies and especially for binging. Do I need to go up to 10 mg? Has anyone else had this experience?

Personal Insights Hiatus and now we're back

So due to life, I ended up taking a 3-4 week break from Zepbound. Prior to the hiatus, my last weigh in on 9/15 was 126.4lbs, and in this 3-4 week time period, I've gained 4 lbs, as of tonight I weigh 130.6 lbs. The last week has been horrendous in terms of food noise, being hungry, eating, and then being hungry or just wanting to snack 90 minutes later. I took my 5mg dose just a little while ago, so I'm looking forward to not being a ravenous fiend. This hiatus was a nice reminder of what life pre Zepbound was like, and I do not want to go back to that; it's all consuming and takes up so much mental bandwidth.

Diet/Health/Exercise Feeling very drained after workouts!

Had a lot of life stressors and wasn’t super regular with workouts this spring.
started zep and workouts this summer - feel extremely sore and drained after the same workouts I did last year.

I have been on zep before and never felt this level of exhaustion.

I also notice mental and physical crashes after lunch - not hypoglycaemia- tracked via CGM. Need a good hour or two to recover.

All other labs came back normal - no thyroid issues or any other glaring deficiencies.

Curious if anyone has had similar symptoms/ experience or thoughts on what I could check or do? TIA

Vent/Rant Intuitive eating and satiety

Preface with, I have had good experiences with some therapists and this isn't a hate post on therapists.

After 15 years of any and all eating disorders at one point or another, I finally sought HAES therapy a couple years ago at my highest weight to heal and move forward with a healthy relationship on food. They stressed SO much about intuitive eating, and frankly it felt like she was angry or upset with me when I would come back and say it's just not working, I'm still binging. She made it sound so easy - listen to your cravings, nothing is off limits, listen to your body's hunger and fullness signals, etc., and then would honestly kind of accuse me of not trying or "buying into" IE.

I'm so mad at them for making me feel like that was due to my own shortcomings and emotional issues. Tonight, I was craving bread, so I had a quarter of bagel and cream cheese. After that, I still was craving something, and opted for a small bowl of cereal and blueberries. I ate 3/4 of that and was full and I finally understand now what IE is - my body didn't want to finish it and I have absolutely no desire for more cereal or a bagel (bagels are my absolute favorite and high craved item!) The craving was there, and I satisfied it until I was full, the end.

I'm mad that I let myself feel that way for years, from a HAES disordered food therapist that clearly never had these same issues. I stopped going to food therapy because she said "if you're not going to get on board there's not really much else." meanwhile I was following the guidance but the cravings. Never. Stopped.

Diet/Health/Exercise Is ZEP better for cholesterol than Ozempic?

Dosing Week 5 on 2.5mg and appetite and cravings are almost back to pre-Zepbound levels

My doctor doesn’t want to increase to 5mg until my follow up in December… What can I do? The first 3 weeks, I lost weight effortlessly and could easily resist cravings. I don’t even feel like I’m getting full easier most of the time anymore.

My average weight loss is still technically 1.3 lbs per week, but the weight graph is really starting to look like I’m hitting a plateau. I don’t want to wait until December paying $300 a month for a med that’s not doing much anymore

Tips/Tricks Dose skipping while on vacation

Just took my third 2.5mg, no side effect and everything is incredible. I have a three-week vacation coming up and I am considering to skip a dose or two so I can enjoy food for some days (I am a strong responder so far every time after my shot I won’t be able to eat for like four days) .
The Lily website says if you miss a dose, you can still have it within four days. So if I intentionally postpone one dose for four days, should I take the next dose at my usual shot day or a week later?

Side Effects Gallbladder suggested to come out now. I’m worried I did this to myself on zep.

I have been having pain just randomly in my upper middle stomach. I was constipated, and I went to the first GP in my practice I could get into which wasn’t my doctor. She told me that she thinks I was just constipated and there’s no way it was my gallbladder, but I pushed to get an abdominal ultrasound while the ultrasound came back and said that I either have a 6 mm polyp or sludge ball. I also have a history of acid reflux and this feels very similar but different. It’s hard to explain. The original doctor still thinks it’s not my gallbladder, but my PCP says it could be so she referred me to a general surgeon. I’m not with him today and he said in his opinion, he would have the gallbladder removed. I’m just so upset because nothing else seems wrong with my gallbladder. My PCP said I could get a HIDA scan to check my gallbladder function, which is basically a test where they do an ultrasound while your gallbladder is pumping to see how it works. The surgeon said a HIDA scan was pointless. I’m not really sure why a scan of how my gallbladder is functioning is pointless when this could be a polyp and not sludge. They don’t know. Everything else looked normal about my gallbladder. I asked him what he was basing his opinion to get it out on and he said that it was based on my symptoms now I’m second-guessing myself if I described my symptoms correctly and it is just acid reflux and they found a polyp just by chance. I mean the original doctor wasn’t even going to do the scan, but I pushed for it. Now I’m having guilt that I did this to myself on Zepbound and ruined my stomach and gut health. I have three kids at home and surgery is not something I can take lightly, especially when I have a heavy baby to pick up and I won’t be allowed to for at least two weeks. I’m just wondering if anybody else had gone through something similar on the bound or had their gallbladder removed or had a sludge ball or anything relative to this experience. It’s just so hard when three different doctors don’t agree.

Achievement/NSV 🎉🥳🎊 Lowest A1C in years

I’ve been on 2.5 since June 12. 5’4 female in mid 20s with insulin resistance and PCOS. Weight has always been a background effect (not something I was striving to lose), and my goals have always been about bloodwork. Lowering A1C and cholesterol. They’re both the lowest they’ve been since I started getting panels done in 2023 :)

A1C is down to 5.2. Cholesterol is still above the normal range but much lower than before zepbound.

I’m so grateful I took the leap. It was scary and challenging but I’m relatively stable in my eating and habits and GI systems now; it was all worth it. 🩷 I will keep going to see what else I can achieve, but this is worth celebrating.

Personal Insights For those who have forgotten to remove the cap, did you get CVS to refill your prescription early?

I have been on Zepbound for about 15 months. I have read several times, over the past year and a half, of people forgetting to remove the cap. I always wondered how that could happen. Tonight, I learned how that could happen. I have severe needle anxiety. Since being on Zepbound, I have had a ritual to get in the right mind. It takes me about 20 minutes. For the first time tonight, I thought I would “just do it.” Unfortunately, I forgot to take the cap off.

For people who have accidentally forgot to take the cap off and ruined one of the precious doses, were you able to get CVS to refill your prescription early?

Vent/Rant I need a hug 🥺

I started this journey on the 4th of July weekend.

I was making good changed eating veggies, more protein, exercising and feeling optimistic. But after prepping for surgery I had last month I was off Zepbound for two weeks. I can't get back on track. The scale is stuck on the same number (I'm glad I'm not gaining)

I just feel like something is not clicking, like I'm not getting the hang of this.

(Currently on 5mg I still have a month in a half to go on this dose.)

I need encouragement, please share what's helped you get over the hurdles?

❤️
