Megathread Discord Server

At the beginning of the year I created a server on an app called Discord, for anyone undergoing Spravato treatments to be able to connect with others who are going through the same thing. Discord is a place for people to connect and talk with each other, and we've created a small community of people all dealing with the same thing. If you'd like to talk to others who are fighting the same fight, or if you have questions about the treatment, here's a link to get you to where we are. You're not in this alone.

What can you not do after treatment?

• Upvotes

I have an initial appointment for spravato scheduled at the end of the month. I’m worried about going because I read you can’t work nor drive for 24 hours which I don’t think I’d be able to find someone to pick me up mid day and even if so, I can’t imagine not being able to work for at least 24 hours after for at least a month. Is this accurate? Also, what else are you not able to do the day you get spravato?

EMDR therapist discouraging Spravato treatment

My med provider (and the one before her) recommended spravato. I’ve been on a med roller coaster for decades and am at a point where I’m ready to try it. With my current schedule, now is the time.

I was approved by insurance but need to work with them about transportation. My state has good healthcare that will provide rides to appointments so I don’t think it’ll be a problem. I hope. The only thing I really see standing in my way is my EMDR therapist.

My prescriber thinks it would be good to try spravato, as does my talk therapist and the PHP/IOP I’ve been attending since March because of trauma. My EMDR therapist is very against it. I can tell she is one that is more into vitamins and natural remedies. I told her I was thinking about it because of tx resistant depression and she basically said that’s a term that is used too freely. I think she thinks EMDR will address depression just fine it’s own.

I’ve been doing EMDR since May and every time she tells me to avoid mind altering substances for 72 hours as we finish up. The first month I was dissociating and having horrible nightmares but that stopped. I had one rough EMDR hangover last month, but otherwise I’m tolerating it really well. No more dissociations, flashbacks or overwhelming distress. It’s making me feel stronger. I’m not overly concerned about spravato reigniting trauma.

This is the first time I’ve ever had providers directly contradict each other. I think it can’t hurt to at least try spravato and I don’t know how to get my EMDR therapist on board. Did anyone have providers who weren’t a fan if it and what did you say to them? And I’m curious what others experience has been doing the two therapies at once.

Thanks!!

Questions/Advice/Support What do you find most effective at covering up the taste?

I’ve been in treatment for over a year and for the longest time had NO problem dealing with any post nasal drip. However, I have vomited my last two appointments and find myself no longer able to cover up that taste.
Jolly ranchers, mints, different types of lollipops, lifesavers and pretty much any candy you can suck on, I have tried.

Does anyone have any other suggestions?

I hate taking zofran bc it gives me bad side effects and really would like to figure out a way to go about this so I don’t have to take it 😭

cognitive effects

anyone notice any cognitive decline on the spravato. i'm 22 f so can't be perimenopause and not on any other medication. i feel brain fog days later and open the wrong cabinet and struggle with word retrieval and remembering things. anyone else struggle with this. i called j&j and they claim they have no heard of this/had guidance but according to this reddit sub and fb group for spravato it is common.

Questions/Advice/Support Title: Spravato (esketamine) for PTSD, depression and derealisation. Could it make things worse?

Hey!!

I’ve been dealing with treatment-resistant depression, PTSD and ADHD. My main symptoms have been severe hyperarousal, hypervigilance, agitation, anxiety, insomnia and episodes of derealisation.

I’ve tried numerous medications, had a lengthy psychiatric admission and undergone ECT. I’m finally seeing some improvements in my mood, but the PTSD symptoms and sleep issues are still pretty rough.

I’ve been considering Spravato (esketamine), particularly for the depression, but I’m worried it could worsen my derealisation, anxiety or agitation given its dissociative effects.

Has anyone with similar symptoms tried it? Did it help with PTSD-related hyperarousal and depression, or did it make the dissociation worse?

Would love to hear both positive and negative experiences before discussing it further with my psychiatrist.

Thanks! 😊

Seeking Empathy/Support where should i go from here?

hey!

so i started spravato about a year ago. only got halfway through the round of treatment before life circumstances meant i could no longer attend spravato sessions and i had to stop (even though it was very helpful!) in that time period, the psychiatrist who was prescribing me the spravato left the practice, so i can no longer go to the clinic i used to go to for spravato.

fast forward to now where i find a new psychiatrist, who im completely unable to get a hold of outside of our appointment times. shes nice and understanding, and said that their inpatient clinic (the only mental institution in our city) does spravato. i was nervous about it being in the same building that you get thrown into for a mental health crisis but i figure ok ill try it, i want to get better.

first day i get there, the first people i see are a police officer, a nurse and a patient being escorted by them. i already dont feel safe. they have a huge no weapons policy (understandable) yet theres a cop walking around with a gun and taser??? no one seemed to pay it any mind. like this was 100% normal. i brought my driver in because at the old clinic you needed to be signed in and signed out. the new clinic just kind of laughed and looked at me like i was crazy and said "oh no you can just come back and leave when youre done" ??????

then the worst part was i was put in a room with multiple chairs, i started to cry (its been a horrible last 2 years) and asked if this was a group session. i didnt even know spravato could be in a group setting. the nurse tried to comfort me but you can 100% fully see and hear everyone, the "dividers" are barely existent. we didnt all start treatment at the same time, and the nurse and patients were arriving and leaving the whole session.

they were all nice people, but were snoring/moaning loudly and it just didnt feel right. i like to meditate during my session and revisit my tramua and try to reprocess it. i literally cannot do that with someone eating popcorn right in front of me. this was the first time ive ever left a spravato treatment feeling worse and now i dont know if i should come back. they scheduled me tomorrow without even asking, and im worried they wont let me cancel (they wouldnt the last time, and you have to call to cancel, the online feature is disabled).

i cant go back to the old clinic because i dont have a psych there. (unfortunately there are a lot of issues with the clinic itself) i found a new clinic that DOES have private rooms, that may be able to get me in (for general psych reasons) monday/tuesday... but its an hour away, and doubles as a weight loss clinic (which im only now realizing is the same one that had a data breach with my information, and im not even a client there...?) i am plus size and dont want to be made to feel ashamed of my weight, so now im worried about this clinic too.

i just dont know what to do. i want to go back to my original provider but shes long since gone. i really liked their setup and i trusted their nurse, but now i cant go back there. do i just give up on spravato? nothing else has helped the way it has, but im not willing to go in a group setting and have a "bad trip" or be with dangerous people that then sets me back even further than i already have been :(

Questions/Advice/Support Anyone have worsening anxiety even after stopping treatment?

I stopped Spravato because it was making my anxiety worse and even taking 30mg of Buspar wasn't helping. My psychiatrist, the nurse who would check my vitals, and the doctor at the office all had never had a patient experience worse anxiety when the Spravato should have been out of my system. September 22 was my last treatment and I'm still experiencing worse anxiety than when I started. I have a very long history of medications not working or making me feel worse (my psychiatrist, a nurse practitioner, recommended that I see a psychiatrist with an MD for a second opinion because I have so much difficulty with medication) but never have I had lasting side effects like this. Has this happened to anyone? Nothing else about my medication changed. I just need to know that I'm not alone. Thank you to everyone who read this.

3rd Session Coming Up

I have my third session for Spravato tomorrow. I am doing it twice a week at this moment. My first session went well, I definitely have no idea what to expect and didn't really know what I should do (listen to, think about, etc). My second session was a bit harder and I didn't really vibe with the music that I had on. I felt very anxious the whole time until it started to wear off a bit. I also found out after my last session that my long term partner ghosted me and moved to a different country, which is definitely something I don't even know how to begin to process. I think in a way it's a godsend that I'm starting this treatment after all of that happening, but I am wondering if there is something particular I could try to do during my sessions to help me work through some of the trauma and pain from that as well.

Any suggestions for what I should listen to during or even after treatment?

Anything I should focus on thinking about during/after treatment?

Anything in particular I should be writing/journaling about?

Any suggestions are welcome, I just want to be able to really fully take this time to try to heal and work through some things. I've struggled with depression, anxiety and suicidal ideation for so long and I really am just hoping for some relief by trying this treatment. I have hope for it, and I know it can take several treatments before feeling any kind of difference, but I just want to make sure that I am trying to do things to heal through this to the fullest extent that I am.

Questions/Advice/Support First dose today, advice on aftercare and onward?

Hey all 👋

I'm going in for my first treatment right now. Taking the Lyft!

I know about ketamine and how it enhances neuroplasticity. I also know many folks recommend being very intentional with what you're consuming post session.

That said, as is, I often am very addicted to screens and will come home from work and either watch movies or play video games. I don't want to do that tonight. Honestly, I'm hoping it just relaxes me and I can sleep.

Anyway, I'm curious what y'all would recommend for aftercare? What do you usually do after a session?

Are you mindful of what you consume?

I've been playing the Witcher 3, for example, but idk that I want to do that after ketamine because the world is pretty dark and depressing.

Maybe I should get some analog hobbies, like puzzles or drawing.

Ideas for aftercare, during care, and onward appreciated :)

Oh, and I may or may not have CFS/ME. For those who know what this is and have tried spravato, I'm curious if there are any unique challenges we face or advice you may give to someone like me.

Appreciate y'all

Questions/Advice/Support Can I workout later?

So I’ve been doing spravato treatments for around 2 years consistently and I have gotten to the point where it doesn’t really even give me much brain fog or anything. Even during treatment I’m completely able to have a normal conversation the entire time. I’ve been getting back into working out this last year and have never had the desire to workout after treatment but I really feel like I could use a pick me up and just switched to a new gym I can walk to from home. So I’m wondering if anyone has tried working out after treatment and how it’s gone if so?

TLDR: is it a bad idea for me to go to the gym a few hours after treatment

This is one that I enjoy for the most part. Does anyone else have a selection or playlist to share? It’s been a lot of trial and error to figure out what works and doesn’t (not just what you listen to) to get the most out of my treatments.

I am so lost without a fan for example.

Questions/Advice/Support Consultation Preparation

I’ve been in a very dark place for probably the past six months or so. I’m going in for a psych evaluation in December, but I’m pretty sure at the very least I have very severe OCD and likely autism as well (my psychiatrist brought this up to me, I honestly didn’t see the signs until he pointed it out). Until I can get a diagnosed, it’s hard for me to get the kind of meds and therapy I need.

My psychiatrist brought up Spravato to me because the clinic I go to offers it. insurance covers it since I’ve tried *so* many antidepressants, and at this point, I’ll give anything a shot. But I’m going to be honest, I have no idea what to expect. All I can get from my psychiatrist is “it helps”.

What are some good questions to ask during the consultation so I can better understand what my experience will be like? Or advice from people who are currently using it? I’m not necessarily nervous, I just have no idea how it’s going to feel. Any guidance would be appreciated!

Questions/Advice/Support Has anyone been able to overcome aphantasia while on Spravato?

I'm taking Spravato for depression, but I had the thought that it is literally mind altering, is it possible it could help with aphantasia? I know my aphantasia is contributing to my depression. I'd love nothing more than to cure it. Is there hope that Spravato could contribute to curing it?

Questions/Advice/Support Strange dreams

I have had two doses of spravato so far, and it's definitely helping but I've noticed every night now I have extremely vivid and strange, not necessarily scary, just very bizarre and real feeling dreams. I typically do not remember my dreams at all so this is new for me. I plan to ask the treatment team next time I go about this, but wanted to hear from other people. Has this happened to anyone else? Is it a coincidence?

Questions/Advice/Support Smoking

So I’ve been taking my mom to her appointments for a while now and the last 3-4 months she’s been feeling a lot higher than ever before. (She’ll tell you she’s a product of the 70’s so she enjoys it) At first we thought it was because she was on prednisone but she finished that 2 months ago. I realized today that it coincides with when she quit smoking. Anyone else notice that they get “higher” after quitting smoking cigarettes?

From Beautiful to Mild to Minimal

I'm confused and pretty bummed. My first treatment (84 mg) was such a beautiful experience. I think I felt love and joy for the first time in my life (42 yrs). I continued feeling completely symptom free for about 48 hours.

My second treatment was a mild experience. While I didn't feel bad, I didn't feel better.

I just had my third treatment today and that was a very minimal experience. Just felt a little fuzzy and honestly laid there bored. I checked with the Dr and left after about 30 - 40 minutes.

I am already on Wellbutrin, Lithium, and dextromethorphine - all know to augment Spravato. The doctor said there are additional things we can do for augmentation but it's a little too early for that yet.

Are there people who just don't have the typical experience? Can one build a tolerance that fast? The first time was so healing and seems like it would be such an important part of the treatment.

Questions/Advice/Support Spravato and adhd meds?

So I have a meeting tomorrow for intake to do spravato. I know I’ll discuss this there but I want to hear your thoughts on it too. I’m seeking treatment for depression I’ve been dealing with for years, my psychiatrist talked to me about trying spravato so I was going to give it a shot.

There’s a complication however, in that today I was diagnosed with adhd. I’ve been seeking a diagnosis for a while but didn’t expect to have the results line up with my spravato intake before I could have a psychiatry meeting.

Those of you with adhd, is it okay to start meds for adhd if you’re on spravato? What if taking adhd meds improves my depression because I can function more normally? How do I know what is helping? I have a week before I see my psychiatrist as well, any advice would be wonderful no matter how tangential.

Questions/Advice/Support First treatment - worried and nervous.

I am getting my first Spravato treatment in 5 days time on the Sunshine Coast in Australia. I am worried and nervous. They have told me that I will be starting on 24mg. That doesn’t sound like a lot compared to some of the other posts I have read. Do you have any advice for me please?
