It's not common but can happen, a slight mechanical error caused a cascade problem the result of which my car is currently trapped between 1806 Earth, Mars in 2256, and a random phone box in 1973. It's a pretty big deal with ripples in spacetime radiating out from the collision point. Does my insurance cover this? Given it's a mechanical error that I couldn't have known about I was cleared from blame.
