Career query How do I get back into workforce after 4 years career break?

Location: southern USA.
I took a career break 4 years ago because of some health issues in family. Now thankfully everything is back to normal and I’m actively looking for jobs.
Throughout my career I’ve kept my knowledge sharp and learned new skills. Even got a few certifications to stay relevant to my field.
I’ve been looking for work for a the past 6 months with no luck. I lowered my desired annual salary by 10-15k. Which didn’t make much difference.
At this point Im considering working at a minimum wage/ internship r just to get my foot in the door. I genuinely miss working and want to get back into my field.
For those who have rejoined the work force after extended career breaks, how did you do it? What are the strategies that worked for you? I’d greatly appreciate any advice.

I have hands on independent project as shown in the resume. My technical skills are at best intermediate and I continue improving my skills during free time. But I always wonder why my resume doesn't even pass the screening and I don't get any interviews.
