One of the most common questions in FTM top surgery is:
“Which technique is right for me?”
There is no single technique that is best for everyone.
The choice usually depends on several factors, including:
• Amount of breast tissue
• Skin excess
• Skin elasticity
• Nipple and areola position
• Chest width and anatomy
• The patient’s priorities regarding scars, contour and nipple sensation
Keyhole is generally considered when there is very little breast tissue, minimal skin excess and good skin elasticity.
Periareolar may be suitable in selected patients with limited skin excess where some adjustment around the areola is needed.
Double incision provides greater control over skin removal and chest contouring, so it is commonly used when there is more breast tissue or significant skin excess.
An important point is that cup size alone is not enough to choose the technique.
Two people with a similar chest size may require different surgical approaches because their skin quality, nipple position and chest anatomy may be different.
A final recommendation should always be made after an individual surgical assessment.
