How do you get a tattoo consent form signed before the client sits down?
Short answer: build the tattoo consent form as a mobile form the client signs on their own phone, either from the booking link before the appointment or on a shop tablet right as they arrive. The clipboard and the paper version are the slow part of the whole process.
We build form software and get this question from shops a lot, so here is what a tattoo consent form has to capture to hold up for both the artist and the client:
- Full legal name, date of birth, and an age check. Once you collect the date of birth, the form can confirm the client is 18 or over instead of you squinting at an ID in low light.
- A medical and skin history section (allergies, blood conditions, recent sun or medication). Conditional fields keep these questions hidden until someone answers yes, so most clients fill out a short form.
- Placement, design notes, and an aftercare acknowledgement, so the client confirms they read the care instructions.
- A real signature field. The client draws it with a finger, the same thing a tattoo release form or body art consent form does on paper, except it attaches to the record for you.
The signature capture itself does not need a paid plan in the tools we have seen, which catches people off guard since they assume e-signature is always an upgrade. What usually costs money is turning the signed form into a saved PDF for your files, so plan for that if you need a filed copy.
One honest caveat: a form is not legal advice, and a tattoo waiver meant to hold up in a dispute should still be reviewed by someone who knows your local rules.
How are you handling the signing step right now, paper in the shop or something digital?
