It's overdue for a trim and has an infected central sulcus. Is the horse lame?

Needs a trim bad and gas thrush

The bars and sole of the hoof haven’t shed off. There is also central sulcus thrush. I highly recommend packing the crack between the heel bulbs with Artimud. Iodine will be helpful too to dry it out and kill the thrush.
