Housing accommodation

I have an appointment with my doctor and cardiologist coming up and I want to get all my paperwork together to apply for accommodations since it will likely be the only time I can get in with either of them before my program starts (Nevada healthcare tends to book out 3+ months) I found the paperwork to go over with them for work accommodations but I haven’t found anything for what I need to gather for housing accommodations- what paperwork/letters would be best to request from them?

You need a letter on their office letterhead outlining your needs and how best they can be accommodated . 

What accommodations are you looking for?

Ideally a solo room, I come with a lot of medical aids (forearm crutches, shower seat, i keep a rolling cart with my medications and things) so space is big, also I need to be able to take naps and get extra sleep when I can, its really important for my health, im hoping since i have diagnosed and documented disabilities backed up by multiple doctors they’ll take it seriously

Based on what you are sharing here, I'd also put in the request to be put in an apartment with a stall shower. The majority of the apartments have them (all at West and the majority of East based on info from Flamingo), but a handful at East have bathtub showers and those are kinda tall/can be difficult to get in and out of for some.
